InfoWell - Take Control of Your Health
The InfoWell Diabetes Portal provides health information and educational tools for St. John's Rehab Hospital patients with type 2 diabetes. This secure website is created by Shared Information Management Services (SIMS) in partnership with health care professionals and patients.
The InfoWell Diabetes Portal can help patients better understand, stay healthy with and manage their diabetes, providing:
- Information about type 2 diabetes and its treatment, medications, exercise and blood glucose monitoring
- Practical information about healthy eating, food portions, low-fat eating and label reading
- A diary to record daily food intake, blood glucose test results, and personal feelings
- Information about other health conditions such as high blood pressure, heart and circulation problems, kidney disease, and certain eye conditions
