St. John's Rehab Hospital is responsible for personal information under its custody or control and has designated an individual, the Corporate Privacy Officer, who is accountable for St. John's Rehab Hospital's compliance with the following principles.

Through written materials, signage and discussion with hospital personnel, St. John's Rehab Hospital will identify the purposes for which personal information is collected.
