As a patient of St. John’s Rehab Hospital, you
have been referred to us by an acute care
partner or physician. Whether you were brought
to St. John’s Rehab to recover from a traumatic
injury, cancer, cardiovascular disease, stroke,
organ transplant or other condition, we are
committed to maximizing your recovery and
making the most of your stay at the hospital.
St. John’s Rehab’s team of medical specialists and rehabilitation professionals will work with you and your family to ensure that your individual rehabilitation goals and needs are met. Typically, you will be discharged or referred to the appropriate outpatient rehab program once your rehabilitation program is complete and your individual goals are met.
