Back on Track
Individualized
and Goal Oriented Therapy
Back on Track
is an outpatient rehabilitation program for clients who have been involved in traumatic
motor vehicle and work-related accidents. We help patients
progress towards their pre-accident level of function in their work,
activities of daily living, mobility, leisure, and social roles.
St. John's Rehab Hospital's client-centred, multi-disciplinary care approach encourages clients to participate and succeed in their rehabilitation. The Back on Track team facilitates this process by ensuring:
- Cost-effective rehabilitation
- Time-line driven treatment plans
- Evidence-based practice
- Best practice guidelines
- Assistance in reaching case resolution
Established Reputation
The Back on Track program has been meeting the needs of our clients and their
families, physicians and insurance companies since 1995. In January 1998,
Back on Track was awarded a Workplace Safety and Insurance Board contract
to assess and treat workers who have sustained work-related burns, amputations
and complex trauma injuries. This endorsement of Back on Track has demonstrated
its ability to deliver excellent cost-effective care that is responsive to
clients and referring sources.
Expertise
Our interdisciplinary team of rehabilitation specialists are experienced
in treating clients who have sustained traumatic injuries such as:
burns, amputations, complex orthopedic, musculoskeletal, and mild to
moderate acquired brain injuries. Our directly relevant experience
allows us to set both realistic rehabilitation time lines and to comment
on maximum medical recovery, and its influence on function.
