Interdisciplinary
treatment
Back on Track provides clients with individualized, cost-effective, goal-directed and time-line driven rehabilitation. Treatment protocols and equipment are current and we continually evaluate our rehabilitation programs through best practice guidelines and goal-directed team rounds.
Monthly reports provide details on recovery in the areas of vocation, avocation, mobility, physical status, activities of daily living, psychosocial presentation and transportation. Clients are also enrolled in the Road to Recovery Group, a nine week psycho-educational group to educate clients process and resolve the issues resulting from a debilitating injury. Access to the heated hydrotherapy pool is also available.
Single discipline treatment
Should a client require only single discipline treatment, this is what will be recommended and provided. The same up-to-date equipment and treatment protocols will be used and rehabilitation will focus on return to pre-accident level of function.
Physical conditioning/work hardening program
As a client's abilities and independence improve, this program is necessary to prepare for return to work and to increase physical and cardiovascular fitness. Physical demands of the client's job and homemaking requirements are simulated in therapy to allow the client to return to these activities gradually and safely.
Burn, amputee and physiatry clinics
On a regular basis, the client, the team and the specialty physician meet to discuss the client's progress and how we can facilitate further improvements. Issues discussed include current level of recovery, content of the rehabilitation program, client's level of participation, upcoming surgical interventions, and plans for return to work and full community reintegration.
